Slow MARPE, the DOME approach
MARPE Palatal Expansion Without the Gap, Knutsford
Palatal expansion has evolved, and so has Dr Piekara’s approach. At Kclinic in Knutsford, we now practise slow maxillary expansion using the DOME philosophy: an airway-focused treatment that widens the upper jaw gradually, builds new bone, and works in synchrony with clear aligners, so there is no gap between your front teeth throughout the process.
What is MARPE and Slow Maxillary Expansion?
MARPE (Miniscrew-Assisted Rapid Palatal Expansion) has long been the gold standard for expanding the upper jaw in teenagers and adults. At Kclinic, Dr Piekara has refined this further, moving away from rapid expansion towards a slow maxillary expansion protocol following the DOME philosophy. Rather than forcing the jaw open quickly and managing the consequences afterwards, slow expansion works with the body: gently stimulating new bone growth, building a stable wide arch form, and, crucially, working simultaneously with clear aligners so there is no unsightly gap between the front teeth at any point during treatment.
A more precise, more comfortable approach to palatal expansion
- Custom-made MARPE appliances with lab-fabricated jigs for precision-guided screw placement
- Slow expansion protocol, building new bone gradually across the palate for greater stability and comfort
- Simultaneous aligner treatment using the no-diastema protocol, meaning tooth alignment begins once the suture has split
- Piezosurgical cortical split available where greater resistance to expansion requires additional support
Why choose MARPE in Knutsford?
- No gap between your front teeth
- No extractions, no surgery in most cases
- Easier, deeper nasal breathing
- Better sleep quality and focus
- Fewer in-person visits with remote monitoring
- Fixed brace option also available
- Airway-focused orthodontics, treating the root cause

The MARPE treatment journey at Kclinic
1.
Initial consultation
We discuss your concerns, goals, and medical history to understand whether MARPE and slow maxillary expansion is the right path for you.
2.
Full assessment
A comprehensive evaluation including CBCT 3D imaging gives us a precise picture of your jaw structure, airway, and suture maturity to plan your treatment accurately.
3.
Guided TAD placement
Titanium mini-implants (TADs) are placed using precision jigs fabricated by our laboratory, ensuring optimal positioning. Your aligner treatment plan is also reviewed and approved at this stage.
4.
MARPE placement and aligner bonding
Your custom MARPE appliance is fitted and aligner attachments are bonded, all in a single visit, minimising the number of appointments from the outset.
5.
Expansion phase
Slow, gradual expansion follows the DOME philosophy, gently building new bone across the palate. Aligner treatment begins only once the suture has split, keeping your smile gap-free throughout.
6.
Active expansion phase
With the arch widened and bone stable, your aligners bring your teeth into their final position across a broader, healthier arch, without extractions.
Smarter Monitoring with DentalMonitoring®
Using the latest remote tracking technology, we keep your treatment on course, with fewer visits:
- Progress tracked via app
- Immediate feedback
- More convenience, same quality care
Retention & long-term results
Once the desired expansion is achieved, the MARPE appliance remains in place for several months to allow new bone to fully mature and stabilise across the widened arch. The mini-implants are then comfortably removed in-practice once retention is complete.
What happens after expansion?
Smile transformation
Because aligner treatment runs simultaneously with expansion under our no-diastema protocol, your teeth are already moving into their new positions as your arch widens. Once expansion is complete, your aligner treatment continues to refine and finish your smile across a broader, healthier arch form.
Once expansion is complete, we typically follow with:
- Damon braces for full alignment
- Or clear aligners for a more discreet option

The DOME approach
The DOME concept (Distraction Osteogenesis Maxillary Expansion) was developed by Dr Stanley Liu and is built around a simple but profound idea: a dome-shaped palate, wider and more rounded than a narrow, high-vaulted one, creates more tongue space, a larger nasal airway, and significantly better breathing. Rather than focusing solely on teeth, the DOME philosophy treats the palate as the foundation of airway health, facial balance, and long-term wellbeing. You can learn more about the concept at getdomezero.com.
How we apply the DOME philosophy at Kclinic
Dr Piekara has trained extensively in the DOME approach and has made it the cornerstone of his palatal expansion work. Using slow maxillary expansion, custom-made appliances, and the no-diastema protocol with clear aligners, he applies the DOME philosophy in a way that is precise, comfortable, and uniquely suited to each patient. The goal is not just a wider arch: it is a stable, healthy foundation that supports nasal breathing, better sleep, and a smile that lasts.

— frequently asked questions —
Quick answers to your questions
Will MARPE treatment be uncomfortable?
Most patients experience mild pressure sensations during activation. Any discomfort is typically well-managed with over-the-counter pain relief and subsides within hours of each activation.
Who Is MARPE For?
MARPE is suitable for:
- Teens and adults needing upper jaw expansion
- Patients with breathing or sleep issues
- Those seeking crowding correction without extractions
- Patients with narrow arches and facial imbalance
How long does MARPE treatment take?
Total treatment time typically ranges from 4-8 months, including both active expansion and retention phases.
Will it affect my speech?
Minor speech changes may occur initially, but most patients adapt completely within one week.
Can I maintain normal activities?
Yes, with some dietary modifications to avoid hard, sticky, or chewy foods that could damage the appliance.
Will I get a gap between my front teeth?
No. Unlike traditional rapid palatal expansion, our no-diastema protocol runs aligner treatment simultaneously with expansion, so no visible gap develops between your front teeth at any point during treatment.
How is slow MARPE different from traditional MARPE?
Traditional MARPE uses rapid expansion, which opens the jaw quickly and often creates a large temporary gap. Slow maxillary expansion following the DOME philosophy works gradually, building new bone at a natural pace, which is more comfortable, more stable, and avoids the need for aggressive tooth movements afterwards.
When does aligner treatment begin?
Aligner treatment begins only once the palatal suture has confirmed split. This is central to our no-diastema approach and ensures your teeth move into position across a fully expanded, stable arch.
How many in-person appointments will I need?
Significantly fewer than with traditional orthodontics. We use the Dental Monitoring app to track your progress remotely, meaning most check-ins happen from the comfort of your home, with in-person visits kept to a minimum.
